All appointments are self-service. Use our online booking system from your smartphone, tablet, or computer.
How to Book
- Visit our booking portal and select your appointment type and preferred date/time
- Complete all intake forms digitally via PatientAlly
- Submit payment through PatientAlly — at least 6 hours before your appointment

Important: Appointments where paperwork and payment are not completed 6 hours in advance may be cancelled. Please plan accordingly.
If You Have a Medical Issue
Please book an appointment rather than calling the office. Our clinical team will review your concern and ensure you are seen appropriately. For same-day sick visits, book early in the morning — slots are limited.
For life-threatening emergencies, call 911 or go to your nearest emergency room.
New Patients — Ages 16 and Older
We accept new patients aged 16 and older. Everything is completed digitally:
- Book your visit at our online booking portal
- Fill out new patient intake forms via PatientAlly
- Upload insurance card and photo ID (or select self-pay)
- Submit payment through PatientAlly before your visit
We are an internal medicine practice. We do not see patients under age 16. Please contact a pediatrician for patients younger than 16.
Self-Pay Patients
Self-pay patients receive our best rates. We do not deal with insurance disputes — if your insurer is denying claims, we recommend switching to self-pay for a simpler, more affordable experience.
Limited Front Desk
We have limited front desk assistance. Routine questions — scheduling, forms, payments, portal access — are handled online. Our staff is available for clinical questions only.
If you require full concierge-level, front-desk-assisted care, our sister practice offers that service without insurance involvement. Ask at your visit for a referral.
After-Hours
Our on-call provider can be reached after hours for urgent clinical questions through our main line: 720.400.8935. Leave a message and your call will be returned promptly.
